You are invited to take part in a research study exploring the connection between food access and disability. If you choose to participate, you will complete an online survey about your experiences. The survey takes about 15 minutes. Your privacy is very important to us. Your responses will be kept confidential, stored securely, and de-identified so that no one can identify you personally. You may request a summary of the study results after the project is complete.
Some questions may feel personal or uncomfortable. You may skip questions, take breaks, or stop the survey at any time. Participation is completely voluntary, and choosing not to participate (or to withdraw) will not affect any benefits you receive.
